Our new referral process

We are excited to introduce our new referral process. The new online process is designed to help us determine eligibility easier, which means getting supports quicker to those who need it. Our online form has been tested by a wide user group, meets accessibility standards, is multi-platformed and mobile friendly.

Please note that we have incorporated mandatory fields (information that is imperative for a referral to be processed); the ability to easily upload supporting clinical documentation and save your incomplete referral for up to one week. 

From Monday 2 November 2020, we will not be receiving referrals via fax.
